How to Become a Patient
No access to medical insurance? Can’t afford a doctor?
DEO provides free medical care to patients aged 18 and older who meet the following requirements:

No health insurance, including Medicare, Medicaid, private insurance.

Household income within Georgia DPH guidelines.

Residence in Whitfield or Murray County, Ga.

Eligibility interview annually.

At your first appointment allow extra time for paperwork and bring these documents:

Photo ID.

2 proofs of address (rent, utility bill, mail).

Proof of income: recent pay stubs, SSI disability letter, or a signed note from someone providing food/shelter.
OR
A letter from your housing shelter.
