How to become a patient:

DEO serves only patients who have no insurance.

 No access to medical insurance?   Can’t afford a doctor?


DEO treats patients aged 18 and older who meet the following requirements:  


  • No health insurance, including Medicare, Medicaid, private insurance.

  • Household income within Georgia DPH guidelines.

  • Residence in Whitfield or Murray County, Ga.

  • Eligibility Interview

At your first appointment allow extra time for paperwork and bring ALL of these documents:

  • Photo ID

  • 2 proofs of residency (rent, utility bill, mail)

  • Proof of income:  recent pay stubs, SSI disability letter, or if living with someone who provides room and board, a note signed by them saying so.

  • OR a letter from your homeless shelter.
